We DO NOT accept registration for patients living outside the practice area (see practice area map).
To register as a patient with Somerford Grove practice you must:
a) Live in the practice area (a copy of which is available at reception)
b) Be eligible for NHS treatment - this usually means that you are 'ordinarily resident' in the United Kingdom and have been so for more than 6 months - (but reception staff will be able to check your eligibility when you come into the surgery)
c) Provide proof of your identity, eg, passport, driving licence, European ID card, medical card, bank card, Home Office permit to stay
d) Provide official proof of your current address (in the practice area), eg, tenancy agreement, bank statement, utility bill or documentation from a reputable source - in your name and dated within the last 3 months
If eligible to register, we will need you to complete a registration form and bring it with you into the surgery, or it can be completed on the day at reception. You will also be asked to complete a short questionnaire.
Our preferred times for registering patients is between 12:30 and 15:30 Monday, Tuesday, Wednesday or Friday. At other times we may not have the capacity to deal with your enquiry and you may be asked to come back to register.
All new patients over the age of 5 are requested to attend a registration check appointment with our healthcare assistant, as your medical record may not be forwarded to the practice for some time and we need to establish your current health and any ongoing medical issues. Please book this appointment for a registration check when you register.
